goodbye letter to patients

Hi,
I'm an oncology nurse who is leaving to go back and finish my masters. I would like to give my patients a nice letter letting them know I am leaving, giving them the new RN's name and basically thanking them for allowing me to participate in their care. Does anyone have a letter they have used?
