Safe Dosage Clarification

Which of the following medications should be clarified with the doctor?

Ampicillin 100mg/kg/day PO in 4 equally divided doses
Dilantin 100mg PO daily
Flagyl 500 mg IV bolus q3h
Tylenol 325mg PO PRN headache

I say Ampicillin, since safe dosage is max 500mg daily.

What are your thoughts?

I think that since they are all medicines they should be clarified before using.

I think we should take any medicine by suggestion of Doctor. Because only doctor can tell what is best?

The tylenol dose needs to be clarified for a time. It is ordered PRN, but how often does the physician want it given.. Q4H, Q6H etc.
