    Default Vancomycin troughs

    I work on a medical surgical floor at a small teaching hospital that is working on changing their vancomycin ordering and monitoring practices. Currently, we usually get the trough levels 2 hours ahead of time and wait to get the trough results back before hanging the next dose. Just curious about what other nurses or hospitals do because we are now being told otherwise.... Would appreciate any feedback! Thanks!

    Hello combct1,
    All I can suggest you is that all of you should take the decision with each other. Have one meeting and then decide taking others point of view on this problem.
