Guidelines re: Code Blue sheet in OR

Does anyone know if there is a requirement to fill out a Code Blue sheet if the patient coded in the OR, right after induction. The anesthesiologist has a very detailed record of what was done, but i wonder if a code blue sheet would still be required.
Any input or direction would be greatly appreciated.
