    Default Medication Error in a Home Health Setting

    I have a colleague RN in Ohio State who was asking me what to do about a medication error problem he had in one of his client's. I do not know and hope that I can obtain some answers for him through this forum. He mistakenly filled the wrong dose of coumadin in the medi-planner of his client. The error was picked up by another colleague two days later. The patient did not suffer any harm. The patient reported the matter to an agency that conducted an investigation and confirmed that this indeed did happen; and requested the home health agency to do a plan of correction. In addition the agency that conducted the investigation notified the Ohio Board of Nursing.
    My colleague is claiming that he is new in the home health setting and that patient's chart was not updated and was not in this patients home so he filled the medi planner with the information available at the time. This incident happened after the State Board made an unannounced visit and determined that his particular home health agency did not have patients chats with updated information in their homes as it should be.
    I am wondering what is going to happen now that the Ohio Board of Nursing has been notified about my colleague's mistake. Are they going to mount another investigation? What will happen next?

    Hello ,

    as i read out your previous conversation ..I think that Ohio State bored will doesn't take any action towards your friend..Becoz as we are human ..so mistake may be happen to any one...i think they have to forgive him..

    The Board is probably legally obliged to investigate your friend just because there was a report. They will probably ask him about the incident and he can give his side of the story. If the home health agency has been found to not be updating patients' charts, then that is a mitigator for your friend, also. Unless your friend has a pattern of errors, the State Board will likely dismiss the whole thing. It isn't the worst thing to be investigated by the Board; it just feels like it at the time.
