I have been neglecting the blog and site for several reasons. One, the Texas BON requirements have been updated and we had to make changes to our program which took up some time. Two, I am back in school my self and had a heavy semester load, it is going well and I anticipate completing my MSN by June at the latest. Third, new grandbaby on way and daughter in law having Gestational Diabetes and we have had to completly revamp our cooking and eating program to get this under control. She is here with us while my son is doing Desert Training. Lastly, we have been getting some new clinical sites to give the nurses more choices. The latest one to sign agreements is Kindred Medical Center Houston. We have had 5 nurses there so far and things are going well.  I will see you guys in Dallas/Plano class next month. <br />

One of the things this bill will do is create the role of a Nurse Advocate. This is a much needed role in all the facilities. This person will be educated and certifed by TNA to go back to the hospitals and educate and assist nurses with knowing their options like when to invoke Safe Harbor, the process for doing Safe Harbor, the process of Incident Based and Safe Harbor Peer Review, their rights, etc. It is good for nursing and good for the patients we are caring for.</blockquote>

I think this applies especially to the healthcare crisis that we are all facing. I am proud to say that Texas has taken a leap to the front of the class with several new developments. One: The development of a Nurse Advocate role to educate and assist nurses in Peer Review Process, Safe Harbor, Staffing options, etc. House Bill 575 has the support of not only the Texas Nurses Association but, also the Texas Hospital Association. I congratulate them on playing well with others.  :) Secondly, the recent passing of Senate Bill 476 on December 9, 2010. The Texas State Legislature passed Senate Bill 476 that requires the establishment of The Hospital Safe Staffing Law. It requires that all Texas hospitals have a functioning nurse staffing committee. If they do not they are subject to a per day fine. The new rules address the use of Mandatory Overtime with special attention to the use of on call. It does not stop hospitals from using on call but, limits how they use of on call time as a substitute for increasing staffing. This is especially an issue in procedural and surgical areas and the nurse staffing committee must help develop a staffing plan to be sure there is no . The staffing committee must be selected by the nursing peers and 60 % of the committee must be RNs that are involved in direct care.  I believe our Texas Nurses Association and the Texas Hospital Association should be applauded for coming together and finding a common ground to work our staffing issues. This is in the best interests of not only the nurses but, our patients that we are charged with caring for. It is a wonderful beginning. Now, we must all participate in the process and be involved in setting best practices while still meeting the needs of patients everywhere in the facility. Not, just on 1 unit but, in the ER, ICU, Cath Lab, etc so patients get timely care and nurses get rest so they can be at their best and do their full duty to each patient. So, of course I bite and say what’s going on. She proceeds to tell me about a heating sensation starting at her foot, moving up her leg, trunk of her body and arm. States it had happened twice and she could time it. Then, on the third occasion she had the same symptoms but, also a prickling, pin and needle like sensation. So, I proceed to ask about other symptoms and history and tell her this sounds neurological and she should call her doctor the next day and if it happened again to go to an Emergency Department. So, that night her daughter calls to say her mom had the same things but, this time it also went up her neck and into her head and was accompanied by a squeezing sensation. She had her sons take her to a local urgent care center where they triaged her and sent her to the hospital via ambulance. Doctor told them she is having Jacksonian Seizures. Huh? Never heard of that one before? Me either.  CT scan showed abnormal area in brain which they initially believed to be a tumor. She was admitted and had MRI and full body scans. Negative body scan results. Yeah! MRI showed not a tumor but a spindle shaped aneurysm in the brain with surrounding swelling and bleeding. So, neurosurgeon says in a delicate area but, no active bleeding and is surgical. He did a great job of explaining including visual presentation to family the surgery. Recovery plan, risks. etc Surgery went well, minimal blood loss, 3 clips and sutures at anastomosed area and flap. We are on day 2 of recovery and she has been moved to a regular room, catheter removed, and eating soft diet. She is in good spirits, memory, and sense of humor there. Quite a bit of pain, increased with movement even, chewing hurts but, otherwise going well and we expect a full recovery. Jacksonian Seizures  are  a type of partial seizure in which there is no change in consciousness. Patients may experience weakness, numbness, and unusual smells or tastes. Twitching of the muscles or limbs, turning the head to the side, paralysis, visual changes, or vertigo may occur. When the symptoms spread slowly from one part of the body to another, this &quot;epileptic march&quot; has been termed jacksonian epilepsy (first described by Hughlings Jackson).  <br />
So, yes even though neurology is not my practice area. I knew enough to not blow it off as nothing. So, again today I am glad I am a nurse and I still have my dear friend.<br />

Stop and go on vacation. I feel so revived. I rested in Cabana #8 on Half Moon Cay in the Bahamas, parasailed in St. Thomas, rode a catamaran in Nassau. Ate too much &amp; stayed up too late. Only got 1 and 1/2 of my 7 books read. Played a game of Scrabble with a celebrity wife. She beat the pants off me and I love Scrabble. I took Belly Dancing lessons, got a tan, took a holistic cooking class and jammed to the smooth jazz of Boney James, Richard Elliot, Jonathan Butler, Marcus Miller, Candy Dulfer and others. <br />
Wow! It will be hard to top this vaction. Haven't had this much fun since Disney World years ago with the kids. Now, as I get my land legs back and return to work I can give my all to my patients and family without ending up with an empty cup. Pics to follow tomorrow.<br />
Now each of you raise your right hand and vow to take a break at least once a year. This is an order! It is needed for yourself and your patients. If money is an issue, then put on a music CD, fix a cool glass of iced tea, and relax for even an hour at a time and it will help.</blockquote>

The entire office will be closed for Winter Break 1/22-1/31/11. We have never all taken off since we started 9 years ago. It has been  a long road organizing a successful business and I am grateful for the great instructors and support staff who have sewed into the Nurse Refresher Program and my vision of educating and empowering former nurses and creating a venue for them to make a return to competent practice. I have learned so much in this time about not only starting, managing, and marketing a business but, the value of surrounding yourself with good people. I make less now than I have on any job I have ever had but, am the most satisfied I have ever been. When, you are on the right path you just follow where the Lord leads you and you never go wrong. This is the first time I am leaving without my Blackberry or my laptop. You guys pray I do have seizures or something. LOL. I plan to do every legal thing there is to do on vacation. Until, February you guys be good to yourselfs, love your families, and protect your nursing license.</blockquote>
